Off track badly gols and-week-1

I am just going to start fresh again. I am not doing well with my plan of healthy living. I've gained back between 10-15 pounds.  I knew I haven't been eating well, nor was I keeping track of my meals and exercising. The plan to more forward is to eat better and count my carb intake and some form of exercise 6 days a week.
Breakfast should have 45 carbs
Morning snack 15 carbs
lunch 60 carbs
afternoon snack 15 carbs
dinner 60 carbs
bedtime snack15 carbs or 210 carbs per day .
I'm starting on a Monday for weight checks and my daily eating logs, and exercise log
182.6 was the scale reading this morning after breakfast.
Meals-I had 2 servings of cereal and 1/4 cup milk close to the 45. It would have been better to have only 1 serving of the cereal and add a small piece of fruit
Lunch 2 pieces of white toast with nutella , used up all my carbs, but I should have used wheat bread and only 1 added a cheese stick and a cup of veg. soup
Afternoon snack -Almonds they are a good snack
Dinner- Sweetie meatball over brown rice pineapple on the side for dessert we had honey caramel
apples 60 carbs
snack- biscotti 15 carbs
I will be walking  on the days I don't go to the gym, since I hate walking and I have to stop to straighten out the kinks in my back several times hopefully this will motivate me into going to the gym!
